Location:        Barbara Maher, 2322 N Williams Ave.

Hours:            Monday – Friday, 9:00 am-1:00 pm

Wage:             $27.50 Hourly

Eligible for Language Differential

GENERAL RESPONSIBILITIES

As part of the Supportive Housing Team, plan, coordinate, and provide an array of services (participant, family, community service, community building, and social and educational) to permanently housed tenants to improve their stability, quality of community life, and wellness. This includes a mix of traditional case management, skills education, and advocacy duties. Services take place at Barbara Maher apartment complex, a drug and alcohol-free community.This is a non-exempt, represented, part-time position.

SPECIFIC RESPONSIBILITIES

Plan, coordinate and provide an array of social, educational, and community services for tenants to improve the stability, quality of community life, and wellness of individuals and families in low-income and affordable housing.

  1. Collaborate with site management staff, landlords, service providers, and other agency staff to ensure all services are understood and coordinated effectively and consistently; perform or coordinate safety and security function; respond to and intervene in tenant problems and crises identified by housing staff and tenants.
  2. Complete assessments of tenants and link with service providers for further assessment and long term service delivery.
  3. Provide referrals to partners and other area service providers. When needed, serve as liaison between tenants and area service providers/partners to monitor appropriateness of service delivery.
  4. Provide ongoing case coordination and communications or informal arrangements with tenants to improve their housing stability, health and safety and progress towards self-determination; determine additional needs or changes in service or care requirements.
  5. Conduct drug screening for residents as needed.
  6. Respond to questions and inquiries from tenants and assist when crises occur.
  7. Advise, mentor, and train participants, individually or in a classroom or other setting, on housing and community issues such as planning household budgets and landlord/tenant concerns.
  8. Create, develop, and implement community initiatives designed to promote community livability, and assist participants in building informal support networks with other tenants, neighbors, family, friends and the community.
  9. Respond to and intervene in participant issues and crises using community support services and knowledge of participant needs to maintain housing, health and economic stability.
  10. Create and implement participant initiatives designed to promote and build leadership and inclusion in the development of the community. Support participant and community councils, activity committees and other participant and community driven activities.
  11. Perform community outreach functions such as creating flyers, conducting door to door surveys, or planning and implementing events designed to bring together participants, expand public awareness and encourage participation in the program.
  12. Maintain participant logs in designated databases, confidential files and case notes. Prepare weekly, monthly and quarterly reports to meet funding, grant and/or agency requirements.
  13. Perform other data entry/record-keeping functions as directed.
  14. Attend and participate in professional group meetings and trainings; stay abreast of new trends
