What Individualized Care contributes to Cardinal Health

Clinical Operations is responsible for providing clinical specialties support and expertise in the areas of advice and consulting, research and patient care to internal business units and external customers.

The Value-Based Care / Transition of Care Registered Nurse plays a critical role in enhancing patient outcomes and supporting healthcare system performance in value-based and risk-bearing arrangements. This position is responsible for facilitating safe and effective transitions of care across settings, reducing avoidable hospital utilization, and promoting chronic disease management. The nurse collaborates with patients, families, providers, and interdisciplinary teams to coordinate care, address social determinants of health, and ensure adherence to evidence-based protocols and value-based care goals.

Responsibilities:

  • Transitions of Care Management

    • Proactively identify and follow up on patients discharged from hospitals, skilled nursing facilities, or emergency departments.

    • Conduct telephonic or in-person post-discharge assessments to ensure medication reconciliation, follow-up appointments, symptom management, and patient understanding of care plans.

    • Coordinate timely follow-up with primary care or specialty providers and close care gaps.

  • Value-Based Care Coordination

    • Support population health initiatives by identifying high-risk patients using risk stratification tools.

    • Assist in the development and execution of care plans for complex and chronic patients aligned with value-based care goals .

    • Monitor quality metrics and collaborate with clinical teams to close gaps in care.

  • Patient Engagement & Advocacy

    • Educate patients and caregivers on disease processes, medication adherence, self-management techniques, and navigating the healthcare system.

    • Address barriers to care, including transportation, food insecurity, medication affordability, and other social determinants of health.

    • Serve as a clinical liaison to promote patient-centered care and informed decision-making.

  • Documentation & Reporting

    • Maintain accurate, timely documentation in the electronic health record and care management platforms.

    • Track and report on performance metrics, transitions of care outcomes, and program impact.

    • Support regulatory compliance and payer reporting requirements related to value-based arrangements.

Qualifications:

  • Active RN license in the state of Wyoming, in good standing

  • Associates Degree of Science in Nursing required

  • Minimum 3 years of clinical nursing experience, preferably in acute care, case management, or primary care

  • Strong knowledge of chronic disease management, transitional care, and clinical quality measures is a plus

  • Proficiency with EMRs and care coordination tools

  • Experience in population health, value-based care models, ACOs, or Medicare Advantage, preferred

  • Case Management Certification (CCM), Care Coordination and Transition Management (CCTM), or equivalent, preferred

  • Excellent clinical judgment and critical thinking

  • Strong interpersonal and communication skills

  • Ability to work independently and collaboratively across teams

  • Detail-oriented with organizational and time management skills

  • Commitment to improving patient outcomes and healthcare value
