Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary
The Qualifying Staff Pharmacist-Outpatient assumes legal and functional responsibility for all activities in their assigned pharmacy including having their name listed on the pharmacy license as qualifying pharmacist. The Qualifying Staff Pharmacist clinically reviews prescriptions and performs final product verification and patient education. This position is responsible for supervising the technician staff to ensure maximum safety, efficiency, and productivity. The Qualifying Staff Pharmacist makes clinically appropriate decisions within their scope of practice under Indiana and Federal law. This position provides clinical services such as vaccine administration, demonstration and education of the use of DME products, and anticoagulant monitoring.
Essential Functions and Responsibilities
- Assumes legal and functional responsibility for all activities in their assigned pharmacy including having their name listed on the pharmacy license as qualifying pharmacist
- Consistently performs clinical reviews of prescriptions by analyzing patient medical records including diagnoses, laboratory values, drug-drug interactions, therapeutic duplications, drug-disease contraindications, and allergies; resolves, documents, and communicates all appropriate clinical concerns discovered during clinical review
- Provides complete patient counseling on all new DME prescriptions; provides counseling on non-DME prescriptions as deemed clinically appropriate; ensures an offer of counseling is provided by technicians on refill prescriptions
- Reviews non-formulary orders for Health Advantage and commercial plans, contacts prescriber, discusses formulary options and educates prescriber as appropriate
- Provides information on medications to physicians, nurses, and other health professionals when requested
- Consistently and accurately enters orders into computer system, clinically reviews all prescriptions, fills prescriptions, and checks prescriptions within professional expectations and guidelines to prevent medication errors
- Evaluates all payer sources to determine coverage eligibility for all patients, consistently chooses correct carrier codes, explores all potential payer sources for patient, makes appropriate referrals, uses all available tools to determine factual financial status, and makes appropriate clinical decisions regarding when to provide medication when financial resources are not available to the patient
- Consistently maintains service expectations while meeting productivity benchmarks associated with staffing model including performing clinical reviews, filling prescriptions, verifying final products, and dispensing medications
- Supervises and directs technicians; provides feedback and training in all work areas to maximize safety, efficiency, and productivity; provides orientation and training as needed for new technicians; actively participates in disciplinary actions when needed
- Maintains a clean, neat, and professional work area at all times and directs staff of technicians to do the same; offers and implements suggestions to improve safety, workflow, and efficiency
- Demonstrates AIDET principles in all communications and resolves crisis situations in a calm, constructive, and professional manner; has the ability to assist others in obtaining positive crisis resolution outcomes
- Maintains efficient work flow by analyzing and adapting to changes in volume and staffing; organizes, deploys, and redeploys staff as needed to maintain flow and utilize downtime efficiently
- Performs duties under a power of attorney document from the organization related to Schedule II medications; orders Schedule II medications and properly processes and maintains invoices of all controlled substances and completion of receipt of online orders; performs controlled substance accountability duties in assigned work area
- Consistently ensures constant state of regulatory readiness by maintaining compliance with all laws, statutes, regulations, and hospital and departmental policies and procedures,
