About the role

Overview

The Blueprint for Health was established to promote high quality care that integrates advanced primary care, specialty care and community-based services to impact Vermonters' health and wellbeing. Advanced primary care encompasses prevention services as well as integration of care and services for people with complex health and social needs. Supported by multi-payer participation, the Blueprint has built a foundation of advanced primary care based on the Patient-Centered Medical Home (PCMH) model and bolstered by multi-disciplinary Community Health Teams (CHTs) that provide care coordination and linkages to services across the care continuum and in the community. Essential to the success of the PCMHs and CHTs is a network of locally hired Program Managers, Community Health Team Leaders, and Quality Improvement Facilitators. This network has been integral to facilitating local transformations and increasing collaboration across community partners.

 

The Blueprint Project Administrator: 
•    Reports to the Executive Director and works collaboratively with all Blueprint Central Office team members, Field Staff, departments at AHS, community partners and other stakeholders to ensure consistent, timely implementation of the Vermont Blueprint for Health. 
•    Provides and supports leadership to promote collaboration and learning between the Blueprint Health Service Area administrative entities, contractors, community partners, Executive Committee, Accountable Care Organizations and other stakeholders to enhance the statewide and local health network by developing, implementing and overseeing in person and remote educational trainings, meetings, including learning collaboratives. 
•    Establishes and maintains accountability to policies and procedures that help local service areas implement and sustain the Spoke System of Care, Pregnancy Intention Initiative and Community Health Team Expansion Pilot including tracking and monitoring of participation, evaluating waiver requests, creation and tracking of pertinent trainings and entering information into the Blueprint portal. 
•    Develops and maintains descriptive materials (e.g. Blueprint Annual Report, Blueprint website) from internal and external data sources to support program clarity and information related to program mission, progress, and evaluation.
•    Communicates to a diverse audience including health care consumers, providers, policy makers and researchers, the legislature and Governor, all departments at AHS, the Director of Health Care Reform and the AHS Secretary. 
•    Be proficient across the Microsoft Suite including Outlook, Power Point, Word, Excel, and Teams so that communication, coordination, scheduling, and education are conducted expertly with internal and external state, federal and national partners. 
•    Writes, revises, and oversees memorandums of understanding (MOUs), grants, and contracts to support core and new Blueprint programs.
•    Demonstrates flexibility at picking up responsibilities and tasks that support the mission of the Blueprint and the Executive Director by aiding with scheduling and communication that support all Health Service Areas.

Environmental Factors

Duties are performed primarily in a standard office setting, but occasional travel may be required, for which private means of transportation must be available. Pressure to meet legislative timelines may occur.
