Program Manager, SDOH - Job Overview

Schedule: Monday-Friday (8AM-5PM)

  • Help expand our Health Advocate Student Internship program, Food Insecurity initiative, and Food Prescription program.
  • Collaborate with primary care providers and community-based organizations to support patients in accessing essential resources.

Key Responsibilities

  • Communicate with sites and train staff on workflows for the Food Insecurity Initiative.
  • Maintain and distribute the FI Resource Guide to patients.
  • Attend regional meetings on Food Insecurity and report updates on FI programs.
  • Develop and revise training curriculum related to Social Determinants of Health.
  • Supervise and mentor a team of university-level Health Advocate Student Interns and AmeriCorps Members.
  • Guide Advocates' professional and leadership development.
  • Manage a team of Advocates, lead training sessions each semester, and complete Field Learning contracts and student evaluations.
  • Provide case management support and evaluate Advocates' performance.
  • Communicate innovations and challenges to the Director of Programs.
  • Ensure compliance with Legacy Community Health standards.
  • Provide direct patient care and advocacy.
  • Support the integration of Health Connect programs into the clinical setting.
  • Build and maintain a local network of resource providers.
  • Operate the case management and care coordination database.
  • Maintain confidentiality per Legacy and HIPAA guidelines.
  • Participate in Legacy’s Performance Improvement Program.
  • Travel between sites as needed.
  • Interview and advise patients on health and illness prevention.

Minimum Qualifications

  • BSW or MSW from a CSWE accredited institution required.
  • Minimum of 2 years experience in social services in a clinic setting required.
  • Preference will be given to LMSW licensure with 2+ years of direct practice experience, LCSW preferred.
  • Excellent interpersonal and communication skills, both verbal and written required.
  • Good knowledge of Microsoft Office suite.
  • Knowledge of public benefits and community resources.
  • Must manage multiple deadlines and decision-making responsibilities.
