Hours: Monday through Friday, 4 PM - 9 PM (Must be available for these hours)

Position Type: Part-Time (25 hours/week)

What You’ll Do:

• Provide personalized care in the client's home, ensuring their comfort and well-being
• Assist with daily living activities, including bathing, dressing, and grooming
• Monitor and promptly report any changes in the client's condition
• Maintain a safe, clean, and supportive environment for the client
• Follow the care plan as directed by the healthcare team, ensuring all needs are met

Who You Are:

• A certified CNA with a passion for home care and a track record of reliability
• Previous experience in a home care setting is highly preferred
• Possess excellent communication and interpersonal skills, with the ability to connect with clients
• Able to commit to the set schedule of Monday through Friday, 4 PM - 9 PM
• Have reliable transportation to ensure punctuality and consistency in care delivery

Responsibilities

• Responsible for assisting clients with either personal care or home management tasks
• This would include assistance with basic personal hygiene and grooming, feeding and ambulation, medical monitoring, and other basic health care related tasks
• Home Management includes tasks that include basic housekeeping, shopping and other related housekeeping tasks

Qualifications

• Minimum age requirements is 18 years
• Valid driver's license, with satisfactory driving record, as defined by agency policy
• Certification, if required by state licensure as a CNA or Home Health Care Aide in the state in which the aide is to provide care
• Completion of a recognized home care aide training program or 6 months of recent experience as an aide
• Must be able to utilize proper body mechanics while lifting and may be expected to lift up 75 lbs.; must be able to sit, stand, and walk for periods of time and bend, stoop, push, pull, and reach
