Primary Care Physician

Overview

Physician Affiliate Group of New York (PAGNY) is adding a Primary Care Physician to the group at NYC Health + Hospitals/Gotham Health, Morrisania. As the largest FQHC in the United States, the Gotham Health Community-Based Practices serve diverse families and individuals throughout New York City. Gotham Health provides care to individuals from varying racial/ethnic, cultural & socioeconomic backgrounds.

Responsibilities

• Primary Care Physician needed to attend to underserved patient population 
• Robust analytic support from Office of Ambulatory Care & Population Health
• Access provided to NYC Health + Hospitals Quality Academy training for emerging leaders
• Support staff on-site inclduing; community health workers, social workers, and community  liasons
• Staff ratio is one Nurse per two providers and a Patient Care Tech per provider 
• Pharmacy on-site for patient centered care 
• Full-time schedule, 40 hours per week, rotating evening clinics and Saturday rotation.

Qualifications

• Board Certification or Eligibility in Family Medicine or Internal Medicine
