Post Treatment Services Support Counselor

Pay range Salary:$29.76-$38.39

Essential Functions:

• Determines eligibility, acceptance, and placement for individuals with Substance Use Disorder issues.
• Serves as an active, supportive team member and adheres to the team approach philosophy in the workplace.
• Works closely with other staff in the implementation and adherence of standards set forth in both quality improvement and utilization management systems.
• Completes and maintains appropriate records which meet the IHS/Tribal mental health and alcohol and drug standards, State of Oregon standards for mental health and alcohol and drug programs, AAAHC accreditation standards, and Program policy.
• Maintains records to meet program documentation, quantitative and peer review standards, clinical and administrative supervisor expectations.
• Participates in outreach, determines eligibility, and makes referrals for individuals who may be interested in residing in the Women’s Transitional Living Center.
• Utilizes crisis intervention techniques to maintain individual and community stability.
• Monitors each individual’s risk of harm to self, others, and property, and refers individuals to appropriate services, department(s) or agencies.
• Provides treatment as reflected in Individualized Treatment Plans that is commensurate with clinician education and licensure as well as individualized resident transition plans.
• Reports and documents possible side effects, medication effectiveness and compliance, and consults appropriately.
• Teaches skills in a group setting in an interactive manner that engages individual participation.
• Works within the Women’s Transitional Living Center. May do other home visits as necessary. Authorized to serve both eligible and non-eligible beneficiaries in an outreach capacity both on and off site. 
• Makes court appearances and provides testimony consistent with confidentiality obligations when requested by the Tribe.
• Summarizes individual’s progress toward goals and objectives.
• Re-assesses effectiveness of and works with team members to adapt treatment plans in a timely and appropriate manner.
• Assesses the need for involvement of family or significant others in the development of the plan of care and communicates recommendations to the treatment team. Assists with implementation as appropriate.
• Provides knowledge of community resources to individuals.
• Coordinates care internally and externally and acts as a liaison with community providers and systems.
• Advocates on behalf of individuals to coordinate appropriate services related to treatment goals and outcomes.
• Facilitates stability and independence, while empowering individuals.
• Participates or facilitates effective discharge planning and treatment closure.
• Actively participates in clinical case staffing, in-service training, and other meetings.
• Operates as lead in maintaining and improving operational workflows of the Women’s Transitional Living Center as well as provides a high level of organization and oversight.
• Performs work in a fast-paced and sometimes stressful environment and must multi-task to complete work assignments.
• Coordinates services such as medical, dental, employment, housing, etc., for individuals.
• Transports individuals in a company vehicle for the purpose of accessing resources in support of transition back into the community, i.e. resources to assist with housing, employment, education, medical/dental care, recovery, and cultural activities.
• Performs case management, assessment of individual needs and provides referrals for services, when appropriate.
• Monitors clinical progress and completes chart forms and progress notes for case file.
• Prepares written status reports to probation officer, courts, and referents and writes and completes weekly reports.
• Participates in weekly clinical staff meetings and weekly individual supervision. Attends scheduled grant team project meetings and assists with project reporting and project evaluation.
• Performs a variety of other duties as assigned by the supervisor.

Education, Experience, and Licenses:

• High School Diploma or GED is required. Associate’s Degree is Human Services or a closely related field of study is preferred.
• Minimum of three (3) years of work experience in alcohol/drug or social services.
• OR equivalent combination of education, training and experience.

Driving Requirements:

• Must maintain a valid Oregon driver’s license or obtain one upon hire, qualify for the Tribe’s vehicle insurance, and provide proof of personal vehicle insurance.
