Responsible for the provision of pharmaceutical care at West Virginia University Hospitals through involvement in direct care, operational, education and administrative activities. Working under the guidance of a licensed pharmacist, the role focuses on assisting with the preparation, dispensing and monitoring of medications to ensure safe and efficient patient care.

The Pharmacist Graduate engages in patient counseling, medication reviews, and supports the pharmacy team in daily operations, gaining the necessary practical experience for registration as a licensed pharmacist.

MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Must be registered as a pharmacy intern, if required by state law or Board of Pharmacy; 
2. Pharm.D from an ACPE accredited school of pharmacy OR; Graduate of an approved college of pharmacy or graduate of a foreign pharmacy who has obtained Foreign Pharmacy Graduate Examination Committee certificate for purposes of getting required intern hours
3. The Graduate Pharmacist must obtain licensure as a registered pharmacist in the state they will provide services prior to the expiration date of the Intern License or within 100 days of hire date.

1. Assumes responsibility, displays appropriate use of authority and accepts accountability in the delivery of pharmaceutical services.
2. Responsible for reading, and transcribing as needed, physician’s orders, preparing medications, delivering medications, and ordering and delivering drug supplies for various patient‑care units within role/scope of practice.
3. Implements appropriate clinical decisions and demonstrates appropriate use of expert peers and or management, as applicable. 
4. Reads physician's orders and prepares medication for dispensing, including unique dosage forms for all patient populations.
5. Has an understanding of standard pharmaceutical/medical terms, abbreviations and symbols commonly used in the prescribing and dispensing of medications.
6. Accurately enters information related to parenteral admixtures, parenteral chemotherapy, parenteral nutrition, and other injectable products in computer systems.
7. Interprets and analyzes reports to replenish patient's medications in the acute care or retail pharmacy setting. 
8. Takes inventory of medications or supplies to replace outdated or deteriorated medications and replace depleted inventory.
9. Assists in the preparation and recording keeping of non-sterile and sterile products, including controlled substances, under the supervision of a pharmacist.
10. Accurately prepares intravenous admixtures, including dilutions for pediatric patients, parenteral chemotherapy, parenteral nutrition, other injectable preparations, and sterile irrigating solutions using aseptic technique.
11. Labels medications, supplies, and patient prescriptions appropriately.
12. Works directly with third parties to adjudicate claims and ensure appropriate patient care up when issues arise. 
13. Assists with medication management therapy and pharmaceutical adherence programs.

14. Works and coordinates with unit floors in discharge patients’ prescription and costs.

15. Performs arithmetic calculations for dosage determinations required in medication and solution preparations. 
16. Performs initial check for medication doses for appropriateness based on weight, age, and other clinical factors.
17. Performs in accordance with departmental standards established for speed and accuracy.
18. Prepares, labels, and maintains records of prepackaged medications after determining usage patterns and prepackaging requirements.
19. Answers telephone and channels calls to appropriate individual(s).
20. Ensures that the pharmacy service areas and drug storage areas in the institution are kept clean and orderly.
21. Participates in planning and implementing change and maintaining and improving productivity through attendance and participation in staff meetings, committees, task forces and projects.
22. Assists in the education of pharmacy students, pharmacy residents, new pharmacists, new pharmacy technicians, and pharmacy interns.
