We’re looking for you.

The Pharmacist is responsible for staffing the LMH Health System Pharmacy. The LMH Health System Pharmacy is an outpatient pharmacy that is designated as “own use”.  The pharmacy serves LMH Health patients; as well as, medical staff, employees and their dependents.  The pharmacist works collaboratively with providers, nursing, social work, and inpatient pharmacists to meet the needs of patients.

Responsibilities

  • Process prescriptions for LMH Health patients, employees, medical staff and their dependents.
  • Vaccinate patients under collaborative practice agreement.
  • Educate patients regarding appropriate prescription and over-the-counter medication use.
  • Facilitate transitions of care through the Meds to Beds Program.
  • Provide medication therapy management (MTM) services to eligible LMH Health plan members.
  • Precept pharmacy students on APPE rotations and oversee employed interns.
  • Serve as a drug information resource for prescribers regarding mediation use in the outpatient setting.
  • Actively engage in professional development to maintain current skills and knowledge of clinical standards of care in the outpatient setting.

Qualifications

Required:

  • Kansas Pharmacist License.
  • PGY1 residency or equivalent experience (3-5 years of clinical pharmacy practice).
  • BLS Certification.
  • Certified immunizer.

Preferred:

  • Board Certification or intent to complete within 1 year of hire.
