JOB DESCRIPTION:
The full time night-shift clinical pharmacist position of 7 days on/ 7 days off, or equivalent, will provide care in the areas of emergency medicine (EM) and critical care (CC). The EM/CC night clinical pharmacist will work collaboratively with our night shift clinical/staff pharmacist and EM and CC pharmacist clinicians and pharmacy residents. This position will be responsible for the provision of comprehensive pharmacy services for adult and pediatric patients in the emergency department (ED) and adult critical care patients in our medical and surgical ICUs. Duties will include: clinical order verification, monitoring and evaluation of pharmacotherapeutic regimens, response to drug information questions, patient counseling, pharmacy student/resident education, coordination of operational functions with pharmacy technicians and stroke alert and trauma/code response.
EDUCATION:
Doctor of Pharmacy or Bachelor of Science degree from an ACPE accredited school of pharmacy required. ASHP accredited PGY1 Pharmacy Residency and ASHP accredited PGY2 Pharmacy Residency in Emergency Medicine or Critical Care or equivalent experience preferred. Licensed pharmacist in the state of Vermont.
EXPERIENCE:
Completion of PGY2 Pharmacy Residency in Emergency Medicine or Critical Care, OR completion of PGY1 Pharmacy Residency plus 2 years of equivalent experience, OR 5+ years of equivalent experience preferred.
