Qualifications
Summary:
Under the administrative supervision of the Director of Case Management and Peer Support Services, the Office Based Case Manager is responsible for the delivery of case management services for a designated caseload of child and adolescent or adult priority clients. The Office Based Case Manager is under the clinical supervision of the Clinical Director or designee.
Job Responsibilities:
- Provides case management services and completes related documentation in compliance with established criteria and time frames for such services.
- Provides office visits/phone calls to clients, as needed, in order to link, monitor, and assess their overall functioning in their natural environment but in compliance, at a minimum, with the contact requirements of the level of the client’s case management.
- Is responsible for maintaining a minimum of 30 individual billable contacts each month as well as manager med only clients.
- Participates in the initial individual program planning process with all service providers (i.e., therapist, medical staff) and provides ongoing information regarding client progress to the service providers.
- Provides assistance to clients in the utilization of community resources in the areas related to independent living skills (i.e., facilitating payment of rent, locating appropriate housing, or purchasing food).
- Provides transportation for clients, when no alternative transportation is available, to appointments for medical, counseling, and other essential services in order to facilitate the client's remaining on outpatient treatment status.
- Completes an individual program plan with each client within 7 days of enrollment into case management.
- Writes a minimum of 90% of progress notes collaboratively with case management clients. Completes all progress notes in accordance with the Center’s procedure on Time Frames for Clinical Records and Related Documentation.
- Maintains documentation of a six-month service update in the clinical record.
- Completes a case management assessment prior to the development of the individual program plan.
- Participates in the development of a crisis/safety plan for priority consumers.
- Participates in regularly scheduled Peer Review.
- Is involved in the initial intervention in crisis situations with case management clients and coordinates the provision of other needed crisis services 24 hour per day, seven days per week, and 365 days per year.
- Participates in weekly group supervision with a focus on client-specific and practice-specific activities on an individual basis with the case management supervisor until the supervisor determines that the case manager’s skill level warrants less intensive supervision.
- Facilitates and assists with Telemedicine services and needs of clients and CAREY.
- Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies.
- Is expected to have regular and predictable attendance and the ability to work cooperatively with others.
- Other assigned duties by supervisor or management to meet the needs of the organization.
Position Qualifications:
Must have a minimum of two years’ experience as a case manager and graduation from an accredited college or university with a minimum of a bachelor’s degree in a health-related field of counseling, psychology or social work, or must be licensed as a nurse (LPN or RN). LPN’s must have fifteen semester
