Position Summary

The Home-Based Acute Care Occupational Therapist is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for inpatient hospital at home patients in the home setting in accordance with established policies while maintaining the highest degree of quality patient care. As a member of the multi-disciplinary team the therapist assumes responsibility for communication of patient-related issues with appropriate team members or other staff and participates in collection and evaluation of data necessary to assess appropriateness of care at home. The Occupational Therapist may also be assigned to orient staff and students. The Occupational Therapist works with the patient and family/caregivers in the home setting without direct supervision.

  Demonstrates clinical competency in evaluating, planning, and delivering quality therapy services, including assessment, intervention, discharge planning, and documentation. Assesses clients' physical, cognitive, and emotional status, as well as home safety and caregiver support, to develop individualized care plans in collaboration with the multidisciplinary team.

  Provides therapy in accordance with physician orders, modifies treatment as needed, and documents all services per agency guidelines. Educates clients and families on treatment goals, home programs, adaptive equipment, and community resources. Ensures coordination of care, communicates effectively with internal and external teams, and participates in discharge planning.

  Manages a caseload independently, prioritizes urgent needs, and communicates schedules appropriately. Provides supervision to COTAs and home health aides, participates in staff education and in-service programs, and contributes to agency planning and compliance. Maintains confidentiality and adheres to HIPAA, infection control, and safety regulations across all settings of care.

  Education and/or Experience

• Required - Bachelor's Degree In Occupational Therapy from an
• institution approved by the state board in collaboration with the American Occupational Therapy Association.
• Required - 1 year Therapy experience with adult geriatric and/or individuals with physical disabilities
• Preferred - 1 year Therapy experience in home care or in related rehabilitation setting
• Preferred - Experience with the geriatric population

License and Certification

• Active and unencumbered Occupational Therapist license in state(s) of employment required.
• Required within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed)

Skills

• Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
• Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions.
