Nurse Practitioner, Physician Assistant –Expanded Access Primary Care I
Indian River
Cleveland Clinic Indian River is seeking an experienced provider (FNP or Adult APRN or PA) to join the Primary Care team in an innovative care model that is focused on opening ease of care for patients.
Partnering with a physician and another APRN/PA, the team works three days a week in person and one day virtually to manage patients' overall health, diagnosis new illnesses and manage chronic diseases.
Locations can vary on a day-to-day basis: Based out of PCTC and HWC
Schedule: Monday-Friday, 7am-5:30pm, Saturday 8am-3pm
Three days a week in person and one day virtually
Rotating four 10-hour shifts with alternating rotating Saturdays
No Holidays,
On-call for their patient panel.
Prefer 2 years of Primary Care Advance Practice experience

APRN: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to an APRN protocol and specialty certification. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems.
PA: Nationally certified licensed healthcare provider who provides primary and/or specialty medical care
(the practice of medicine) in a variety of healthcare settings. Functions within the collaboration agreement of a Physician, as per regulatory requirements. Performs complete histories and physicals on patients of all ages to establish and record patient's current health status and to develop a working diagnosis and treatment plan to provide continuing medical care, including daily examinations, writing orders, chart notes, discharging patients and completing discharge summaries. This medical care may be in a variety of clinical settings. PAs in the Hospital care role provide outpatient and/or hospital based care to patients (more than 50% of their time) with complex, acute, and chronic health conditions. Outpatient/Hospital care PAs practice in a setting that requires the performance of diagnostic and therapeutic interventions to manage acute and chronic health problems.
Must have the ability to work with multi-disciplinary teams.
Responsibilities:
- Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
- Must be able to work with collaborating physician.
- Performs and interprets, at least to the point of recognizing deviations from normal, common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as privileged by CCF Credentialing Board.
- Performs routine procedures such as injections, immunizations, suturing and wound care, managing simple conditions produced by infection or trauma, assisting in the management of more complex illness and injury which may include assisting surgeons in conducting operations and taking initiative in performing evaluation and therapeutic procedures in response to life- threatening situations, as privileged by CCF Credentialing Board.
