As RN Nightingale Case Manager, you will use your experience and expertise to mentor newer staff while supporting effective care transitions across settings. This role focuses on patient advocacy, care planning, and empowering patients and families, while ensuring compliance with regulatory and accreditation standards.

A caregiver in this position works PRN (as needed) variable shifts.

A caregiver who excels in this role will:  

  • Support care managers in working a difficult case by using their knowledge and expertise to guide a newer care manager.
  • Perform the clerical functions of Care Management to allow the care managers to perform more transitions and follow up.
  • Document Initial Assessments on all new patients and help patients complete POA or Advance Directive forms.

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited school of professional nursing. 
  • Current state licensure as Registered Nurse (RN) 
  • Basic Life Support (BLS) through American Heart Association (AHA) 
  • 3 years’ experience in an RN Case Management position at the Cleveland Clinic immediately prior to retirement
  • Knowledge of acute care regulatory/accreditation
  • Discharge planning and/or Quality Improvement 

Preferred qualifications for the ideal future caregiver include:  

  • Bachelors of Science (BSN)

  • Requires normal or corrected hearing and vision to normal range.

  • Ability to perform work in a stationary position for extended periods.
  • Ability to communicate and exchange accurate information
  • Ability to work with physical records, such as retrieving and filing.  Ability to operate a computer and other office equipment.
  • Ability to travel throughout the hospital system.
  • Requires normal or corrected hearing and vision to normal range.
  • In some locations ability to lift up to 10 pounds
