What Health System Pharmacy contributes to Cardinal Health
Pharmacy Operations is responsible for the safe, efficient and effective coordination of Cardinal Health's pharmacy operations that service acute care hospitals, hospital retail customers, ambulatory care and alternate site facilities, oncology and cardiology practices as well as retail customers.
Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes.
Job Summary
Medication History Pharmacy Technicians assist the pharmacy, nursing and medical staff in obtaining accurate home medication lists from patients entering the hospital and conveying that information verbally or electronically to the appropriate provider of care. Maintain a professional presence throughout the hospital. Assure pharmacy services and activities are carried out to meet goals and standards of both Cardinal Health and the client/hospital.
Responsibilities
The Medication History Technician will work primarily in the Emergency Department, inpatient nursing units and other patient care areas.
The basic function will be to obtain accurate home medication histories by chart review, patient interview and phone calls to retail pharmacies and physician offices.
Enters complete medication history in computerized system.
Clarifies information through communication with the providers, patients, and other pharmacies.
May use medication history software and state databases to gather home medication histories.
Other duties will include updating computerized home medication databases.
Assists, consults, contributes, executes, and participates in the daily activities of the Pharmacy Department to ensure that optimum pharmaceutical care is provided to all patients.
Works cooperatively with all staff members; voices concerns and suggestions to appropriate persons in a positive manner.
Maintains logs, records, and other required documentation accurately; files documentation in appropriate locations.
Maintains security and confidentiality of passwords.
Attends and participates in departmental meetings, in-services, and quality improvement programs.
Maintains all required training and education per department requirements (e.g. HealthStream, mySpark, Continuing Education).
Adheres to hospital and departmental policies and procedures.
Complies with all requirements related to risk management, safety, security, fire, and infection control.
Complies with all applicable state, local and federal regulations.
Maintains safety-integrity-professionalism of pharmacy at all times.
Organizes and prioritizes work assignments.
Collects data, conducts quality monitors, and completes documentation as assigned.
Demonstrates good oral and written communication.
Answers the telephone, identifying self and department; directs calls to appropriate personnel.
Keeps pharmacy areas clean, neat, and well organized; Keeps pharmacy equipment clean.
Performs other duties as assigned by supervisor.
Qualifications
High School diploma, GED or equivalent, or equivalent work experience, preferred.
Maine Pharmacy Technician license/registration or ability to obtain.
Previous medication history technician experience preferred.
National pharmacy technician certification (PTCB or NHA) preferred.
Flexibility in working schedule.
Ability to work well in a team.
Strong verbal and written communication skills.
Strong customer service skills.
Demonstrated ability to use technology such as computers, smart phones and tablets.
