Title

MAQI2 (Michigan Anticoagulation Quality Collaborative) Abstractor

Responsible for the detailed analysis of medical records

• Collects medical data for a variety of information systems and enters the data into the appropriate system

• Provides the initial data foundation around which the medical staff quality assessment and improvement program

revolves

• Abstracts clinical data to provide the severity adjustment for many outcomes measurements

• Responsible for applying a variety of disease-specific abstraction rules and guidelines to reviewed cases

• Responsible for maintaining the consistency, accuracy and data integrity within all data systems assigned to use

• Undertakes special “projects” or initiatives as warranted

• Responsible for initiating and directing anticoagulation data quarterly submissions and finalizing all data for sign-off by the Pharmacy Quality Coordinator

• Responsible for timely communication with PI teams and leadership regarding abstraction concerns

This role performs analysis of the information present in the medical record and abstracts the pertinent information according to the pre-defined database definitions. Use of critical thinking skills and independent judgment needed to identify and report to leadership additional patient safety issues, or unusual events that may pose risk to the patient and organization.

·         High school diploma or general education degree (GED) is required.

·         Permanent Pharmacy Technician license in good standing with State of Michigan

·         5 or more years of relevant pharmacy experience in a hospital, retail pharmacy, or elsewhere, or equivalent combination of education and experience.

·         Internal applicants must have no current disciplinary action plans and the most recent performance evaluation score above 2.0.

·         Skills in mathematics and basic algebra.

·         Ability to read, interpret and analyze data from various systems and equipment in order to set an appropriate plan of action in coordination with the pharmacists.

·         Work which is constantly and exceptionally tiring mentally and/or visually. Conditions exist more than 90% of the work time.

·         Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects.

·         Job requires effective communication skills to report and discuss findings, as needed with pharmacists, hospital personnel and internal/external customers; ability to foster a positive, professional working relationship with coworkers; self-motivation; accuracy.

·         Must be proficient with multiple standard software(s) on personal computer (Microsoft Word, Excel, PowerPoint, Outlook, SharePoint, and Internet)

·         Must possess excellent interpersonal communication skills and demonstrated ability to independently handle decision making.

·         Ability to stay focused with multiple distractions

·         Effective time management skills with ability to prioritize workload; self-directed
