Sentara Health is seeking to hire a qualified individual to join our team as a

Manager, Post Acute Network Operations

Position Status: Full-time, Day Shift

Position Location: This position is residents of the Hampton Roads area of Virginia (Norfolk/VA Beach/Hampton ) Hybrid work schedule (3 days in office/two out, generally) .

Standard Working Hours: 8:00AM to 5:00PM (ET).

Position Summary:

Reporting directly to the Enterprise Vice President of Population Health, the Manager, of Post Acute Network Operations serves as the subject matter expert and leader for a Post Acute Care Network (PAC) which services patients being discharged across all Sentara Hospitals.

The PAC is a preferred network of Skilled Nursing Facilities, Home Health Agencies, In-patient rehab facilities, Long Term Acute Care facilities, and Hospice facilities which are under participation agreements in support of post-acute care for Sentara Health’s hospitals, Clinically Integrated Network, and Accountable Care Organization.

The Manager of Post Acute Network Operations will lead a team of RNs and Network Operators. This leader will lead contract development and negotiations with Post Acute Care providers across all markets served by Sentara’s hospitals. This individual will work in collaboration with our Post Acute Medical Director to set network quality standards for each site of care (SNF, HHA, IRF, LTACH, Hospice) network; and lead committees for the establishment and exchange of protocols, processes, and practices to improve the care coordination and patient outcomes of all patients discharged to Post Acute Care across all Sentara hospitals and all payers.

It will be necessary for this individual to:

  • facilitate strong relationship management with Sentara Hospital Care Management leaders at all facilities, and the clinical and operational leadership at all PAC Network partners.
  • serve as a system leader amongst the Post Acute Care Medical Directors. Additionally, this role will be a system leader across Sentara in its engagement with CMS programs such as Bundled Payments for Care Improvement and Transforming Episode Accountability Models, as well as all future models related to bundles, specialist, and post-acute care providers becoming incentivized in value-based care.

Minimum Requirements:

  • Master's level degree required. The following fields of study preferred: Business Administration/Management, Healthcare Management/Admin, Nursing, Public Health or Social Work
  • 5 years experience Bundled payment programs for care improvement (Advanced), Chronic Joint Replacement Program
  • 5 years Medicare Shared Savings Program Accountable Care organization
  • 5 years progressive experience in Post Acute Care : Skilled Nursing Home facilities, Home Health Agencies, Inpatient Rehab facilities, Long Term Care Facilities and Hospice Care
