Job Description

Location: Connecticut, Hybrid 

Overview of the Role:

Working closely with our Director, Population Health and Market Medical Directors, you will assist with the management of various elements of Privia’s Population Health team, designed to transition practices into the new world of value-based care. As part of this role, you will be responsible for supporting the advancement and implementation of various strategic population health initiatives and logistics, including educating and engaging Care Centers on how to be successful in population health. 

Primary Job Duties: 

POD (Provider Organized Delivery) Program:

  • Manages the day-to-day activities of the POD and Collaborative program, including curating the content and materials for population health performance meetings, action planning,  and implementing enhancements to the program
  • Owns end-to-end initiatives relating to educating and engaging primary care providers in areas of value-based care
  • Takes lead in strategizing, organizing, and developing content for projects furthering the Population Health team’s core strategies, including but not limited to, Referrals, Documentation Quality and Total Cost of Care
  • Works cross-functionally with Population Health operational leads, Practice Operations, and clinical leaders to foster knowledge sharing and drive behavior change that will improve performance relating to value-based care
  • Manages POD structure in Connecticut market, including maintaining and tracking POD assignments
  • Manages the collection and management of provider feedback on Care Center Transformation initiatives and content to better understand facilitators and barriers to behavior change

Data Collection/Management:

  • Organizes content – and specifically data content – for monthly POD meetings
  • Interprets payer-level performance and works with Population Health operational leads to develop action plans for PODs and providers
  • Manages the delivery of POD-specific data and materials to drive local and provider-level behavior change
  • Builds and manages the reporting of various process measures at the Care Centers-level to measure implementation of key initiatives
  • Leads out on distributing monthly and quarterly population health reports across all markets
  • Builds process documentation and monitor the progress of projects implemented to achieve Population Health’s key strategies
  • Analyzes data to identify issues and trends, especially at the regional- and POD-level, and develop communication strategies to elicit behavior change

Education and Onboarding:

  • Build and maintain relationships with clinical leaders across the market to support them in their efforts to engage and educate providers
  • Develop infrastructure to onboard Care Center stakeholders and clinical leaders and foster their understanding of how to be successful in our value-based care programs
  • Collaborate with other market leaders on the provider education and enablement strategies as it relates to value-based care

Qualifications

  • 3+  years of professional experience in a healthcare consulting or analyst setting
  • 3+ years of project or program management directly working with clinicians including; clinician engagement, clinical workflows, provider relations, clinician education, or practice transformation 
  • MPH / MHA preferred
  • Client-facing experience in clinical integration or transformation activities 
  • Knowledgeable about population health management, value-based care, and payer programs
  • Experience with project management/ task management tools
  • Experience with Excel
  • Ability to work with different levels of clinical personnel including providers
  • Ability to work in a very fast-paced and changing work environment
  • Experience with data collection and document management 
  • Experience coordinating small (1-5 persons) and large scale (20-30 persons) meetings 
  • Must comply with HIPAA rules and
