Touchstone Health of San Antonio is actively seeking a qualified and compassionate LVN to join our great team! The ideal candidate has excellent clinical skills, a passion for direct patient care, and the ability to function well autonomously providing quality in-home patient care in San Antonio and the surrounding area. Responsibilities & Duties:

• Evaluate and treat patients using the most current technology and practices
• Communicate with the physician to decide the initial plan of care based on initial patient assessment & physician’s orders
• Successful management of a consistent case load
• Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
• Manage and educate Home Health Aides in implementing nursing services
• Ability to work in a collaborative environment with an interdisciplinary team
• Ability to travel San Antonio and surrounding area

Requirements

• Current and unencumbered license to practice as an LPN in the state of TX
• Minimum of one (1) year of nursing experience
• Current CPR certification
• Reliable transportation, valid drivers license with no DUI's, revocations, or suspensions in the last 5 years

• Excellent written and verbal communication skills
• MS office skills (Word, Excel, PowerPoint)
