PREFERRED EDUCATION and EXPERIENCE:
High School Diploma or GED
Valid driver’s license, clean driving record, and reliable transportation
2 years of experience working with individuals with disabilities
Ability to engage with others using courtesy, tact, and good judgment
Appropriate safety equipment and first aid kit in personal vehicle
SUMMARY/OBJECTIVE:
Assist and support individuals to maintain health status, develop skills in instrumental activities of daily living, adapt to community and home environments, identify and access community activities and public resources, facilitate relationship building with the greater community, and to empower persons in services to achieve their goals. Support managers with collecting required documentation, provide quality assurance support with medication and money management logs, facilitate activity schedule and planning, coordinate vehicle oversight checks, support individuals with peer-lead activities and committees. #hotjob
ESSENTIAL DUTIES and RESPONSIBILITIES:
Provide support to help individuals learn to be more independent and to meet their goals with variety of day-to-day events. This may include, but is not limited to, bathroom support, household chores, budgeting, making friends, exploring their community, shopping, meal prepping, advocating for themselves, eating, etc.
Demonstrates effective and professional communication with individuals and their families/support systems as well as with providers serving the individuals while establishing and maintaining professional boundaries.
Assist with personal care and Activities of Daily Living (ADLs), including but not limited to directly supporting with baths, oral care, dressing, grooming, etc.
Provide training in skills related to the use of community resources, such as transportation, shopping, and social and recreational activities.
Monitor the health and physical condition of individuals and assist with medication management and other medical needs when applicable.
Recognize and report suspected cases of abuse, neglect, or exploitation.
Supervise individuals to ensure their health and safety.
Provide behavioral support as appropriate and train individuals in adaptive behavior for home and community settings, such as managing anger, handling social encounters with others, and developing a support network.
