Lead Community Health Worker  – 3020 POPULATION HEALTH - HEALTH EQUITY WA SMMC WALLA WALLA at St Mary Medical Center - Walla Walla

The Community Health worker Lead reports to the Manager of Population Health SEWA and promotes comprehensive health and well-being within the community they reside in.  Community health workers or CHWs are an integral member of patient-centered health teams. They walk along side neighborhood residents to achieve and maintain their ideal state of physical and mental health through prevention, education, and access to appropriate treatment. The lead CHW can complete moderately advanced and complex tasks, documentation, outreach, strategy work, and problem solving without direct supervision due to their education or community experience.

Shift: Average 40 hours per week, Day shift, Full-time position. 

Required Qualifications:

  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information
  • Related community experience
  • 1 year experience in healthcare

Preferred Qualifications:

  • Associate's Degree in Health or Social work related field.
  • Applicants who have lived the experience/need

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
