Lead Clinical Liaison RN

This position serves as the Supervisor/Lead Clinical Liaison to the Hub Team(s) and provides leadership and coordination for the care provided to patients and their families referred to home health, hospice, and other post-acute care services that are offered with PHCC. In addition, this position supports, as directed, all Business Development leaders across the Director's full territory. This position is responsible for identifying 'best practices' for care delivery and may network with community resources and consult with case management at the hospital.

Required Qualifications:
- Education sufficient for licensure
- Bachelor's degree in Nursing, or a Diploma in Nursing or an Associate's Degree in Nursing (ADN)
- Upon hire: California Registered Nurse License Or
- Upon hire: Washington Registered Nurse License
- Upon hire: Must be licensed in states wherever care is delivered.
- 5 years Five years' experience in clinical discipline required
Preferred Qualifications:
- 3 years A minimum of three (3) years of staff supervision experience preferred.
- Previous liaison, marketing, sales, or health care industry experience
- 3 years Three years home health, hospice and/or palliative care experience preferred.
