POSITION: Bilingual Lead Case Manager – Unaccompanied Minors Program
REPORTS TO: Assistant Director
CLASSIFICATION:  Full Time (Management Team)
EXEMPT/NON-EXEMPT:  Non-Exempt

GENERAL DESCRIPTION: The Safe Haven for Children Program is a 24-hour shelter and transitional foster care program that serves unaccompanied children (UCs) in immigration custody who are awaiting release to their sponsors.  The Lead Case Manager is responsible for the oversight and execution of all case management requirements for the shelter and foster care programs. The Lead Case Manager may also carry a case load of up to 4 cases as needed. The Bilingual Lead Case Manager is classified as an essential worker, and as such, accepts responsibility for the completion of their critical services to clients, on site, even during times of emergency. These include, but are not limited to, emergencies caused by weather, nationwide pandemics, civil unrest, and domestic threat.

MAJOR DUTIES AND RESPONSIBILITIES INCLUDE:
SUPERVISOR:
•    Represents Lutheran Social Services of New York’s mission, values and ethics; upholds Code of Conduct; and ensures adherence to agency policy and procedures by case management staff. 
•    Enforce all safety rules and regulations in accordance with agency policies and procedures and ensure that UCs comply with established house rules.
•    Provides supervision and support to case managers via individual sessions, team meetings, and in-house training. 
•    Review Portals, charts and Apricot data base for all Shelter and Foster Care cases.
•    Complete timely review of progress notes and Individual Service Plans completed by Case Managers.
•    Ensures quality and excellence in service provision.
•    Participates in weekly team meetings to provide case management updates. 
•    Participates in required LSSNY trainings.
•    Participates in the hiring and training process for newly hired case managers. 
•    Approves timecards of case management staff and addresses any changes in staff schedules. 
•    Participate in a rotating on-call schedule with management team members. 

CASE MANAGEMENT:
•    Assess the needs of and provide support and case management services to all UCs in program.
•    Ensure that Individual Service Plans are implemented, in accordance with all applicable regulations and LSSNY policies, for UCs in care.
•    Ensure that UCs receive essential services such as: medical, mental health, and dental care; basic education; orientation; and legal screening.
•    Identify appropriate parents or guardians to whom the UCs may be released and communicate any concerns regarding potential sponsors with the Federal Field Specialist
•    Screen for human trafficking concerns.
•    Document the provision of services in each UC’s case file.
•    Help UCs identify and screen prospective sponsors.
•    Provide UCs with guidance on activities of daily living, including good personal hygiene.
•    Create and maintain case charts, so they are audit ready.
•    Ensure compliance with local, state, and federal rules and regulations, including but not limited to HIPAA.
•    Document and report all major and minor incidents in accordance with ORR policies and procedures.
•    Provide a stable, abuse- and drug-free environment.
•    Perform other duties as required by direct supervisor or needs of the program/agency.

Qualifications

REQUIREMENTS:
•    Master’s Degree in behavioral sciences, human services or social services fields, OR
•    Bachelor's Degree in behavioral sciences, human services or social services fields AND at least 3 years of progressive employment experience that demonstrate supervisory and case management experience.

•    Bilingual in English and Spanish
•    Strong communication and writing skills.
•    Ability to work collaboratively with consumers and co-workers.
•    Superior organizational skills and an ability to work independently and as part of a team.
•    Ability to exercise enormous amount of patience and good judgment.
•    Resourceful, detail oriented, and able to multi-task.
•    Ability to work flexible hours as needed.
•    Experience and facility with Microsoft Office Applications, including Microsoft Word and Excel
