POSITION SUMMARY:
Adhering to System Values, Service Excellence, and Standards of Behavior, the Inpatient Case Manager leads comprehensive patient care coordination during hospitalization, ensuring safe, timely, and effective discharge while addressing medical, psychosocial, and financial barriers. This role requires clinical judgment, decision-making, and collaboration with interdisciplinary teams to optimize patient outcomes and continuity of care, especially in resource-limited rural settings.
The anticipated hourly range for this position is $27.27/hr - $34.10/hr - $36.83/hr. Final compensation will be determined based on the selected candidate's applicable experience, skills, qualifications, and other job-related factors.
ESSENTIAL QUALIFICATIONS
Education:
- A Bachelor's in Social Work or Human Services, or a related field is required.
Certification:
- Certification in Case Management (CCM) or equivalent is preferred.
- All employees and contracted staff will have either successful completion of an American Heart Association* accredited course in Basic Life Support (BLS), or ACLS and PALS combined. (for clinical staff), Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis.
