MINIMUM QUALIFICATIONS:

• Willing to work with infectious disease patients (STDs/HIV/TB).
• Bachelor’s Degree in social science or health discipline from an accredited college OR a Bachelor’s Degree in any other discipline with at least 6 months of direct case management experience or a Florida Licensed Registered Nurse (RN) with 1-year of direct case management experience or a Master’s Degree in Social Science or Health discipline (which can substitute 6 months of direct case management experience.
• Completion of Medical Case Management AETC Southeast AIDS Education course within 30 days of hire.
• At least 6 months verifiable experience working with electronic medical records systems.
• Successful completion of HIV 500/501 course within 6 months of hire.
• Willing to perform home visits for individuals from diverse backgrounds (varying cultural backgrounds, socioeconomic statuses, and different sexual orientations).
• Ability to bend, kneel, kneel, use fine motor skills, lift and carry a maximum of 25 pounds.
• Valid FL Driver’s License and access to a personal vehicle.
• Emergency Duty.
