Housing Outreach Navigator

Competitive Pay: $20.15-$23.00 per hour, with the opportunity to make a meaningful impact in the community. Job Summary:
Perform outreach services within the {WV, Central, EV} engaging vulnerable individuals experiencing homelessness with a Serious Mental Illness (SMI) designation or those at risk of health concerns. The role focuses on connecting individuals with health and wellness resources, as well as housing services. This position involves conducting outreach in parks, faith-based communities, libraries, nonprofit organizations, heat relief stations, freeway underpasses, and similar locations.Copa Health collaborates with local and state health departments, homeless service providers, emergency management, law enforcement, healthcare providers, housing authorities, local government leaders, and landlords. This role includes obtaining vital documentation for housing services, connecting individuals with behavioral health support, and implementing evidence-based practices. The housing outreach navigator works closely with the Copa Navigation team.

Essential Job Duties and Responsibilities:

• Conduct outreach and navigation services in teams of 2-3, engaging with individuals in parks, libraries, faith-based communities, freeway underpasses, and similar areas.
• Provide information on health, wellness, and prevention services.
• Assist individuals in completing applications for benefits such as AHCCCS, SSI/SSDI, unemployment, and SNAP, as well as housing services, rental/utility assistance, and employment-related services.
• Help individuals obtain necessary documentation, including photo IDs, social security cards, and birth certificates.
• Connect individuals with community-based resources and educate them on available services.
• Work in unpredictable and fast-paced environments with individuals facing homelessness, legal challenges, and/or mental health or substance use issues.
• Oversee program development, monitoring, enrollments, and scorecard maintenance.
• Collaborate with Copa Health programs to provide appropriate short-term and long-term services.
• Offer case management-like services, including referrals and follow-ups, ensuring coordination of care.
• Maintain accurate records of services provided through the HMIS, electronic health records, and intake forms.
• Submit program reports as required.
• Connect individuals who are not enrolled in the Behavioral Health System with Copa Health Outpatient Health Home or other community agencies for assessment.
• Provide various covered services that have encounter value once enrolled in the electronic health record.
• Transport individuals as needed.
• Perform other duties as assigned.

Minimum Qualifications:

1. High school diploma or GED required.
2. At least one year of experience as a Navigator or working with people experiencing homelessness.
3. Peer Support Training certification preferred or willingness to obtain certification.
4. Lived experience in homelessness, mental health challenges, substance use, or trauma preferred.
5. Knowledge of community resources in Arizona.
6. Satisfactory work record as determined by Human Resources.
7. Successful completion of Copa’s training program.
8. Fingerprint clearance card from the Arizona Department of Public Safety or eligibility to obtain one within eight weeks of hire (Good Cause Exception must be resolved before the hire date).
9. Pre-employment drug screening required.
10. TB test clearance required at hire and annually.
11. Valid Arizona driver’s license with state-minimum insurance coverage and ability to meet Copa Health’s driving requirements.
