Hospice Bereavement Coordinator (Hob-PRN)

The Bereavement Coordinator implements planning, direction, and provision bereavement support services for all hospice patients and families to assist in meeting their emotional and spiritual needs during and after the patient's terminal illness.

WHAT YOU CAN EXPECT
Collaborates with other departments within the hospital to align interdepartmental functioning, goals, and expectations as outlined in the SLA.
Conducts comprehensive bereavement assessments to identify bereavement strengths and needs of patient and family; evaluates grief needs and assesses risk factors associated with grief.
Conducts follow-up with grieving families to ensure their support needs are met and to gain insight into their satisfaction of care; communicates regarding bereavement support via newsletters and sends cards of condolences as appropriate.
Conducts preparatory meetings with the patient and family before the patient's death and maintains availability during the bereavement period as needed.
Documents patient care information and ensures timely and accurate completion of patient care records in accordance with policies and procedures.
Periodically plans and organizes memorial services for Hospice families.
Provides support and counseling consistent with patient and family beliefs.
Reviews and develops protocols for treatment and proposes current options for interventions; provides educational in-services for the interdisciplinary team related to Bereavement support.
QUALIFICATIONS
Master's Degree- Social Services or Divinity- Required
2 years- Social Services Experience- Required
2 Years- Chaplaincy Experience- Required
2 Years- Counseling Experience- Required
