Position Summary
• Responsible for patient identity management, provider medial record deficiencies notifications and medical record form design.

Education
• Associates degree or
• High School Diploma or equivalent and applicable experience
Licenses and Certification
• N/A
Experience and Skills
• Technical Experience: High School Diploma or equivalent with 3 years experience in clinical or technical office support in healthcare environment required
• Associates Degree - no experience required
• Proficient with computer applications with advanced MS Word, Excel
• Proficient in analysis of information and making sound determinations based on analysis
Physical Activities
• Intermittent hand manipulation required
• Intermittent lifting and carrying of 20 pounds
Role Specific Responsibilities
o Manages the patient identification systems in SIH HIS systems by identifying, researching and correcting wrong or duplicate patient account and/or medical record numbers.
o Collaborates with SIH Medical Group patient identification specialist in managing (identifying, researching and correcting) the NextGate EMPI
o Identifies and notifies providers and stakeholders of incomplete and delinquent medical records.
o Tracks and communicates provider delinquency status to stakeholders
o Designs medical record forms in Word for placement in Optio Forms System.
Compensation (Commensurate with experience):
