Job Summary:

Working under the supervision of a RN supervisor, the LPN Care Manager is responsible for coordinating services within the health center and providing support and management to health center patients. The LPN Care Manager will participate in delivery of care, coordination of referrals, and provide comprehensive care management and follow up. This professional has a well-developed understanding of community resources and can multi-task efficiently and effectively while providing personalized, compassionate care. The LPN Care Manager conducts assessments of patient/family needs for the highest risk patients or those patients at risk for poor outcomes across the care continuum. The LPN Care Manager collaborates with multidisciplinary care team members, formulating a documented care plan that ensures continuity and goal setting to equip individuals and their families to manage chronic conditions, easily access resources, adopt healthy lifestyles, and improve health outcomes.

Experience:

  • Three years of clinical experience as a Medical Assistant (MA) and/or Licensed Practical Nurse (LPN) in at least one of the following settings: case management, disease management, home health care, hospital, or physician office setting and Two years of prior electronic medical record (EMR) experience.

Education:

  • Graduate of an accredited Licensed Practical Nursing Program required
  • Bachelor's degree in Nursing preferred

License and Certifications:

  • LPN Licensure in PA
  • Basic Life Support (BLS)

Skills Needed:

  • Ability to incorporate strategies for interacting with persons from diverse backgrounds.
  • Effective behavioral and educational strategies such as motivational interviewing, teach-back method, and self-management support.
  • Ability to set priorities to coordinate care plan efficiently.
  • Ability to recognize the role of cultural, social, economic, and behavioral factors in accessibility, availability, acceptability, and delivery of public health services.
  • Ability to work both collaboratively and independently.

Essential Functions:

  • Provides comprehensive care management services for a designated population/patient panel of up to 100 patients
  • Provides care management for the highest risk patients to reduce risk and emergency department utilization to improve outcomes
  • Provides coordination of care for the highest risk patients experiencing a transition to or from care facilities and/or providers
  • Monitors patients in person or by telephone
  • Collaborates with all levels of the clinical team to improve processes, communication, and team skills needed to provide the best care to patients
  • Facilitates comprehensive care plans in coordination with patient/caregivers and provider(s)
  • Facilitates patient engagement, providing frequent follow up
  • Promotes patient self-management, provides education about disease processes, and empowers patients and families to achieve maximum levels of wellness and independence.
  • Provides interventions using evidence-based techniques such as motivational interviewing
  • Evaluates medical, rehab, psychosocial socioeconomic, psychological, and physical needs and makes referrals.
  • Provides medication management, including medication reconciliation during care transitions
  • Identifies issues related to the healthcare system, transportation, financial, and/or psychological barriers and works collaboratively to problem solve, coordinate services, and to advocate for patients/families
  • Utilizes computer databases and documentation systems and develops and maintains accurate case records of all patients
  • Performs patient care activities within scope of licensure, elevating clinical needs to RN or provider per health center’s processes and procedures
  • Performs other duties as assigned
