Position Overview

The Care/Case Management (CM) Lead is responsible for the development of staff, criteria, guidelines for systems to effectively operate a comprehensive Care/Case Management Program. Provides leadership and supervision to Care/Case Managers in the Medical and Behavioral Health staff of Charleston, West Virginia office for Acentra Health. Assesses needs and plans, communicates, and designs services that are appropriate for the West Virginia Fee For Service Members and Providers.

Job Responsibilities

  • Assessing, planning, implementing, monitoring, and evaluating actions required to meet the client’s health and human services needed. Integrates and coordinates services using continuous quality improvement tools such as Audits for the CM team.
  • Use of Social Determinants of Health (SDOH), any additional CM resources available, and the Atrezzo system.
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered goal planning by developing individual care plans.
  • Handle case assignments, draft service plans, review case progress and determine case closure through discharge planning with CM Team.
  • Develop effective working relations and cooperate with medical team throughout the entire case management process. Arrange abeyances with social services, health, and governmental agencies.
  • Record cases information, complete accurately all necessary forms and produce statistical reports. Promote quality and cost-effective interventions and outcomes.
  • Will be accountable for the full patient care cycle.

Required Qualifications Experience and Skills

  • Minimum of 5 years postgraduate of an accredited school of nursing and currently licensed as a Registered Nurse in the state of West Virginia.
  • Preferred to have Supervisory or Leadership Management
  • Preferred to have a minimum of 5 years’ experience in Case Management position.
  • Must have analytical ability for problem identification, assessment, and review of data/statistics obtained from on-going Case Management process.
  • Certification in Case Management and BSN preferred.
  • Effective and compassionate communication skills.
  • Excellent organizational and time management skills.
  • Familiarity with professional and technical emerging knowledge.
  • Problem solving skills and ability to multi-task.
  • Strong computer skills, knowledge and navigational skills.
  • While this position offers flexibility with remote work, individuals must be based and reside within 50 miles of the West Virginia border.
