About the role

Job Summary
Under the general direction of the Primary Care provider or Registered Nurse (RN) designee, the Licensed Practical Nurse (LPN) is responsible for providing necessary and appropriate nursing care within the scope of license, patient education. The incumbent will prioritize patient care needs, ensure accurate clinical data and work collaboratively and efficiently with the assigned Clinic care team.

Essential Job Duties & Responsibilities
▪ Provide nursing care to assigned patients
▪ Provide educational materials to patients
▪ Work with assigned Medical Assistants to ensure timely, efficient care
▪ Document patient data and review for accuracy

Other Job Duties & Responsibilities
▪ Prepare patients for examinations and procedures.
▪ Perform patient laboratory testing in accordance with patient clients. Clean, arrange laboratory and exam room equipment, order medical supplies
▪ Administer medication or injections under supervision
▪ Assist the providers with various procedures
▪ Using clinical reasoning, conducts accurately focused assessments according to practice standards and LPN Scope of Practice
▪ Reports data to assigned provider and assist the RN in developing plan of care and formulation of goals/outcomes for the patient and family needs.
▪ Participates in evaluating the quality and effectiveness of nursing practice, including, but not limited to, nursing sensitive indicators.
▪ Caring Practices: Creates a compassionate, supportive, safe and therapeutic environment for patients, families and staff. Participates in management of both emotional and physical pain with the aim of promoting comfort and healing and preventing unnecessary suffering.
▪ Recognizes, appreciates, and incorporates a patient's unique differences, such as culture, spiritual beliefs, gender, race, ethnicity, lifestyle, socioeconomic status, age, and values, into an individualized plan of care.
▪ Advocacy and Moral Agency: Preserves and protects the confidentiality, autonomy, dignity and rights of patient and family. Identifies/reports and participates in resolving ethical and clinical concerns.
▪ Actively participates in Clinical Team Huddles
▪ May be required to travel between clinical sites
▪ Assign or give directions to Medical Assistants, based on workload, patient care needs
▪ Other related duties as assigned

Job Qualifications
▪ Current and unrestricted LPN Licensure –State of Ohio
▪ IV certification preferred
▪ Phlebotomy experience strongly preferred
▪ Minimum one year’s experience in a Health Clinic setting with patient load of
▪ 16-20 patients a day.
▪ Working knowledge of EPIC and/or electronic health record software system.
Physical Demands/Work Environment
This job operates in a clinical environment. The role will require the use of personal protective equipment (PPE) and use of appropriate medical equipment (blood pressure cuff, thermometer, scale, and sharps). The LPN will be required to sanitize and dispose of medical supplies as Clinic OSHA and CDC dictate.
