Job Title: Care Coordinator - Outpatient Clinic
Mount Sinai Health Partners (MSHP) is the management services organization developed to implement the Health System’s population health initiatives: programs of care that enhance the individual patient’s experience and health status, improve the health of patient populations, and reduce utilization and healthcare costs. MSHP provides the infrastructure and machinery to support population health initiatives, particularly preventive care, through care coordination, information technology, workflow optimization, and quality reporting. The Department of Social Work Services is positioned to take a lead role in the development of MSHP by providing transitional and longitudinal care coordination services.
The Care Coordinator is an integral member of the interdisciplinary care team and is responsible for coordinating care and providing guidance to patients. The Care Coordinator closely collaborates with the patient’s care team to develop and implement a comprehensive care plan and associated interventions. She/he interacts with patients and family caregivers, as appropriate, to ensure continuity of care, adherence to care plans, and identification of barriers preventing adherence to care plans and interventions. She/he utilizes different behavioral change modalities while coaching patients and family caregivers on how to make lifestyle behavior changes that can positively impact a patient’s health. In consultation with the patient’s care team, the Care Coordinator provides health education related to symptom management and preventive care and advocates to ensure that patients receive necessary care and services.
LOCATION: This position is on-site and is based in the IMA clinic at 17 East 102nd Street, Manhattan.
- Bachelor’s degree.
- Experience in healthcare or related field is preferred.
- Proficiency in MS Office. Ability to learn hospital documentation and scheduling systems and care coordination applications.
- Excellent oral and written communication skills and ability to foster a supportive environment for patients.
- Bilingual, English/Spanisd, preferred.
Minimum of one year in current position.
Compensation Statement
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $58,661.00 - $79,823.56 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
