FHCSD’s Program for All-inclusive Care for the Elderly (PACE) is designed to help our participants maintain a high standard of independent living. Our program provides a wide range of supportive services including outpatient care, meals catered to dietary needs, medical equipment, transportation, social services, and social activities. We are currently seeking a Registered Nurse Case Manager to provide care for our participants as part of an interdisciplinary treatment team.

Job Duties

Provides case review to patients within assigned community health program, ensuring the medical program is clinically correct and being followed.
Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; in addition, ensures that all client files are kept up to date with RN signature on required documents on a monthly basis.
Completes Chart Reviews, ensuring medical program requested is being followed.
Ensures applicable regulatory and department standards are adhered to on an on-going basis (The Joint Commission, OSHA, etc.).
Performs other duties as assigned.
Reviews and sign-off on all Case Managed client Care Plans, Initial Assessments, and Comprehensive Assessments.

Requirements

1 year of experience in community health and in providing human services to medically underserved, high risk, and/or disabled populations.
Current California Registered Nurse licensure. Nurse Practitioner, Physician Assistant, or Medical Doctor licensure accepted.
Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving with 7 years

Desired Skills and Abilities:

Ability to work effectively with patients from diverse social, cultural and economic groups.
Ability to work well in both a team-based environment and independently.
Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.).
Bilingual in English/Spanish preferred.
Demonstrated ability to be culturally sensitive and respect diversity.
Excellent organizational skills, attention to detail, time-management skills, and strong motivation to meet deadlines and achieve goals.
