We’ve got a great opportunity for a Clinical Care Specialist position located at Youth and Family Services at 1012 W. Abriendo Avenue
What You’d Be Doing
As a Clinical Care Specialist, you would serve as a key partner in promoting mental health and overall wellness for individuals and families in the community. This role focuses on building strong, supportive relationships while guiding clients through challenges and helping them develop skills for long-term stability. You would work collaboratively with treatment teams and community partners to ensure clients have access to the resources and care they need, whether through counseling, skill-building, or connecting them to vital services. By combining clinical expertise with a compassionate, client-centered approach, you would help individuals navigate complex systems, overcome barriers, and achieve their personal goals for health and wellbeing.
As a Clinical Care Specialist under the supervision of the Director and Supervisor you will:
• Provide behavioral health counseling, group treatment, and resource-based case management
• Offer consistent support and availability until clients achieve improved functioning
• Teach and enhance coping skills, symptom management, problem-solving
• Provide education regarding mental health diagnoses
• Identify stressors, and increase the use and involvement of natural supports
• Develop, implement, and review individualized treatment plans
• Make appropriate referrals for mental health and substance use needs
• Collaborate with other treatment providers to ensure a team-based approach
• Offer community resources, including peer support activities
• Help ensure clients attend scheduled appointments; provide reminders and arrange transportation as needed
• Maintain accurate, complete, and up-to-date records according to Health Solutions standards
• Maintain competency and proficiency with the electronic health record system
• Perform other job-related tasks as assigned by the director or designee
Physical requirements include the following:
• Requires sitting for prolonged periods of time
• Occasionally requires driving
• Occasionally lifts and carries items up to 20 pounds
• Requires normal range of hearing and eyesight to record data electronically and communicate with clients
• Occasional exposure to communicable diseases and bodily fluids
• Requires occasional driving in adverse weather conditions
• Requires frequent use of hands and fingers to operate a computer keyboard and mouse

QualificationsWhat We’re Looking For—The Must-Haves
Baccalaureate degree in social work, psychology, or closely related field.
Experience working with children/families.
Valid Colorado driver’s license and access to transportation during working hours, and computer proficiency required.
Demonstrated computer proficiency
Exceptional flexibility, time management and documentation skills
What We’d Like to See in You—The Nice-to-Haves
Currently pursuing or holding a master’s degree in social work, clinical psychology, or a closely related field
CAC eligible or completion of CAC training
Spanish language proficiency
Experience working with individuals with severe and persistent mental illness and/or co-occurring disorders
Background in community systems navigation and connecting clients to resources such as housing, food assistance, and peer support programs
Prior experience providing case management services to diverse populations
Demonstrated success working within multi-disciplinary teams and collaborating with community partner
