Position Summary:
The Travel Registered Nurse/Case Manager provides comprehensive care to patients through assessment, planning, and direct nursing services across multiple service areas. This role requires frequent travel to patient homes and agency locations within an assigned region and supports coverage needs due to staffing gaps, PTO, or census fluctuations.
.Required Experience for Travel Nurse/Case Manager:
- Currently licensed registered nurse in the State of Michigan.
- Minimum of 1 year of nursing in home care setting preferred.
- CPR certification preferred.
- Excellent communication and organizational skills.
- Compliance with accepted professional standards and practices.
- Self-directed with the ability to work with minimal supervision.
- Flexible, reliable, and responsive to the needs of dynamic and changing assignments.
- Position Requires High Travel
- Must have reliable transportation and a valid driver’s license.
Accountability for Travel Nurse/Case Manager:
- Accountable for timely response and coverage of patient visits in designated areas as assigned.
- Adheres to HIPAA privacy and security standards and all other applicable regulations.
- Provides care and services in accordance with professional standards of practice, Plan of Care, and physician orders.
- Develops and updates patient-centered care plans in collaboration with the interdisciplinary team.
- Maintains open communication with patients, caregivers, physicians, and health care team members.
- Ensures seamless transitions of care across markets and disciplines.
Responsibility for Travel Nurse/Case Manager:
- Is responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured or infirm, maintenance of health, promotion and prevention of his or her patient’s illness.
- Maintains the clinical record for each patient receiving care.
- Administers medications and treatments as prescribed by the patient’s physician or podiatrist.
- Acts as the coordinator of the health care team in order to maintain the proper linkages within a continuum of care.
- Prepares and maintains clinical documentation according to Agency policies and acceptable nursing standards.
