Schedule: Monday–Friday, Day Shift 0800-1630 or 0830-1700 (Remote/Hybrid options available)
Position Summary:
Overlake Medical Clinics, LLC (OMC) is currently seeking a detail-oriented and compassionate Registered Nurse (RN) to join our centralized nurse team. We are located on campus in building 5 (Starbucks building) floor 6.
The Centralized Nurse plays a vital role in managing electronic communications between patients and healthcare providers, primarily through the Epic MyChart system. This position is responsible for assessing patient symptoms and concerns via phone or electronic messaging, determining the urgency of care, and directing patients to the appropriate level of service. The role supports timely, effective, and coordinated care while reducing unnecessary emergency room visits and enhancing clinical efficiency.
This is a hybrid position. Upon successful completion of onboarding and training, you will be eligible to work remotely (work from home). However, attendance is required for all in-person staff meetings, educational sessions, and any other on-site activities as determined by the supervisor. During the initial onboarding and training period (approximately the first month), you will be expected to work on-site five days a week.
Key Responsibilities:
Patient Assessment/Triage
Evaluate patient symptoms and medical history via phone or electronic communication.
Apply clinical judgment and standardized triage protocols to assess urgency.
Accurately document assessments and interactions in the Electronic Health Record (EHR).
MyChart InBasket Management
Monitor and respond to patient messages, including medical inquiries, refill requests, orders, and documentation.
Follow established guidelines for message triage, escalation, and provider communication.
Care Coordination
Direct patients to appropriate care settings (e.g., emergency department, urgent care, primary care).
Schedule appointments, coordinate referrals, and ensure follow-up on patient needs.
Collaborate with physicians, specialists, and interdisciplinary care teams.
Patient Education
Provide guidance on symptom management, medication adherence, and home care strategies.
Educate patients and families on treatment plans, procedures, and chronic disease management.
Clinical Documentation
Utilize SmartPhrases and other Epic tools to streamline documentation and communication.
Collaboration
Work closely with physicians, advanced practice providers, and other clinical staff to ensure continuity of care.
Compliance & Quality
Adhere to clinical protocols, HIPAA regulations, and organizational quality standards.
Participate in quality improvement initiatives, staff training, and workflow optimization.
Qualifications:
Education: Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
Licensure: Active RN license in the state of practice.
Experience: Minimum of 2 years in a clinical setting, preferably in emergency, inpatient, or urgent care environments.
Certifications: Current CPR certification required
