JOB SUMMARY:
This role is part of the clinical care teams responsible for triaging Grand Itasca patients over the phone to direct appropriate care while supporting the clinic providers through patient care coordination before, during, and after clinic appointments, both in-person and virtually. Clinical Care Team RN is assigned to group or provider panels and be an ongoing resource between patients and providers for patient care management. This role assists clinical care teams with any patient care requiring an RN. One or more days per week, the RN will be schedule in and dedicated to the nurse procedure schedule where they are directly accountable and responsible for managing a patient schedule for procedures within the RN scope along with injections and immunizations. This position requires critical thinking skills in making decisions regarding patient care which includes knowledge and integration of relevant standards, resources, and data.
QUALIFICATIONS:
- Has a current RN license to practice nursing in the state of Minnesota.
- Demonstrated knowledge of the Minnesota Nurse Practice Act and the Scope of Ambulatory Care Nursing.
- Demonstrates clinical competence in providing direct patient care and patient and family education as well as assisting the physician in the delivery of patient care.
- Has current BLS certification (or is willing to obtain within two months of employment.)
- Demonstrated ability to manage the complex needs of a family medicine practice.
- Demonstrated competence in managing patients using Patient Care Protocols.
- Ability to manage change in a positive patient centered manner.
- Clinic experience preferred.
- Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision and mission.
- Maintains reliable and consistent attendance.
