This is a Fulltime, 40hours a week, non-exempt position that will be working in our Milford-Congress location.
This position is eligible for benefits and has an hourly rate starting at $20/hr.

Duties & Responsibilities:
• Dedication to ensuring individual health and safety, willingness to respectfully collaborate with the interdisciplinary team (nursing, clinical, management, residential directors, health care providers, guardians).
• Physical assistance with ambulation, physical assistance with all activities of daily living (showering, toileting, incontinence care, dressing, meal preparation in accordance with individual dietary guidelines, CPAP set up), proper use and documentation of assistive devices (motion detectors, bed rails, video/audio monitor, gait belt, helmet, shower chairs), training and implementation of individual specific protocols (seizure protocols, vagus nerve stimulator use, head banging protocols, CPAP protocol, dietary guidelines, ADL guidelines, fall protocol, constipation protocol), accompanying individuals to medical appointments with the ability to provide pertinent information and advocacy .
• Supervises and transports clients during assigned hours, this may include, job search, medical appointments, weekly recreation, shopping trips, and other necessary trips.
• Supports and teaches clients new skills including, but not limited to, those specific to goals stated in the Individual Service Plan.
• Provides a positive, supportive role model for the client.
• Administer medications.
• Documentation and timely submission of all progress notes, weekly summaries, attendance, and all other client documentation deemed necessary by the company.
• Attends all weekly, monthly, etc. meetings specific to the client.
• To support individuals in attaining the highest quality of life while maintaining their safety across all settings.
Job Qualifications:
• High School Diploma or GED
• Strong verbal and written communication skills.
• Valid driver’s license and reliable transportation that can accommodate individuals safely to their destination if needed.
• Able to drive house wheelchair vans with individuals.
• Ability to act appropriately in the event of an emergency.
• Must be able to pass required certifications in the allotted time set forth by the agency.
• Map Certification must be completed with 6 months of hire
• Must pass a criminal and driver’s check.
• Basic computer skills (Microsoft Office, Internet, Databases), copy, scan, and fax.
• Ability to work shifts to accommodate business needs.
• Must be willing to accompany individuals in the community (Religious services, hospitals, etc.)

• Social Justice Services (SJS), supports those leaving incarceration to make positive, sustainable life transitions through residential and non-residential programs.
• Behavioral Health (BH) has worked to break cycles of violence, crime, and incarceration by providing client-centered interventions and support services to empower and strengthen individuals, families, and communities. BH provides domestic violence treatment and education, community reentry services, mentoring programs, and intensive outpatient programs.
• The Crime and Justice Institute (CJI) is a non-profit organization working at the local, state, and national level to improve public safety and the delivery of justice. CJI provides technical assistance, research, and other services to improve outcomes in policing, the corrections system, pre-trial, re-entry, community supervision, and juvenile justice.
