The RN Care Manager role collaborates with team members to provide care management and expedite essential care coordination support to high-risk patients to improve health outcomes and avoid unnecessary emergency and inpatient admissions. Services may be performed in person or telephonically. Assessments and interventions are recorded in the electronic health record.
