Rural Health Transformation Regional Developer – Eastern Region

Grant funded through September 30, 2028 (position is contingent upon renewed grant funding by the terms outlined by contract through September 30, 2031). Responsible for Great Bay CC, Lakes Region CC.

SCOPE OF WORK:

Performs outreach, partnership development, program coordination, and technical assistance activities associated with the Rural Healthcare Transformation (RHT) grant, with a focus on strengthening rural healthcare workforce capacity and access to care in the Northern region of New Hampshire. This position works across healthcare providers, education and training systems, workforce partners, and community-based organizations to develop and implement sustainable workforce and care delivery solutions that address critical shortages in rural healthcare settings. Hybrid position with a designated home base at an assigned Community College System of New Hampshire (CCSNH) college campus. Regular travel throughout the assigned region and across the state is expected to support employer engagement, partner collaboration, student support services, workforce initiatives, and grant-related activities.

ACCOUNTABILITIES:

  • Works with the Regional Lead to build and sustain a regional rural healthcare hub, developing partnerships with:
    • Rural hospitals, critical access hospitals, and health systems
    • Long-term care and behavioral health providers
    • Community health centers and public health organizations
    • CCSNH colleges, training providers, and workforce partners
  • Supports the development and implementation of rural healthcare workforce strategies that address regional shortages, including:
    • Career pathways and training pipelines
    • Work-based learning models (e.g., apprenticeships, clinical placements, on-the-job training)
    • Incumbent worker upskilling and retention strategies
  • Provides technical assistance to healthcare employers and partners to design and implement workforce and training solutions tailored to rural environments, including small-provider capacity and resource constraints.
  • Engages employers and partners to identify barriers to workforce recruitment and retention (e.g., housing, transportation, childcare, burnout) and supports development of responsive strategies.
  • Collaborates with High School Healthcare Pathways Developers and education partners to strengthen rural talent pipelines, including exposure, dual enrollment, and transition to postsecondary training.
  • Coordinates with CCSNH faculty, staff, and Workforce Development Offices to align programming with rural healthcare needs, licensing requirements, and credential pathways.
  • Assists the Workforce Director at the assigned college location(s) as needed to address and resolve grant-related issues, communicating when required with the CCSNH Director of Workforce Development and the Rural Health Transformation (RHT) Grant Manager.
  • Supports the Workforce Director at the assigned college location and participates, as needed, in statewide, regional, and federal RHT-related conferences, seminars, meetings, and workforce initiatives to ensure programmatic efforts remain aligned with best practices, evidence-based models, and statewide workforce development strategies.
  • Supports innovative approaches to care delivery and workforce models in rural settings (e.g., cross-training, team-based care, telehealth support roles where applicable).
