The Heart & Vascular Navigator will function as a central figure in the overall operations of the Disease Specific Program/Center (i.e. Valve Center, Heart Failure Center, Prevention and Wellness, Chest Pain). The Navigator is a Registered Nurse who works closely with the department directors, managers and staff who have direct or indirect responsibilities for the program. Coordinates daily operations of the program/center including effective communication and collaboration of program activities with physicians and the multi-disciplinary team. Expected roles include direct patient contact and coordination of care ensuring optimal care and patient experience as well as efficiency for patients with cardiovascular disease (i.e. Valve disease, Heart Failure, Prevention and Wellness, Chest Pain). Must demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
Qualifications
Education, Credentials, Licenses:
Graduation from a bachelor's degree nursing program.
Current registration to practice nursing in the state where work is being performed.
Obtains required CEU’s, including all state required courses.
Specialized Knowledge:
Extensive knowledge of nursing standards and practice in their clinical area.
Excellent oral and written communication skills and an accomplished facilitator.
BLS (Basic Life Support)
ACLS (Advanced Cardiac Life Support) = Chest Pain Coordinator only
Kind and Length of Experience:
Five years’ experience in Nursing or related field.
