RN Utilization Review *Hybrid* - PACE/Elderplace

About the role

This is a hybrid position that would require some traveling in the Portland Metro area.

Provides comprehensive case management designed to assure the quality of life for Providence ElderPlace (PEP) participants who are admitted to PPMC emergency department or inpatient care, other hospitals. Communicates daily with ElderPlace primary care providers (PCPs) about status of hospitalized participants; makes rounds as needed to assess status, progress and discharge needs; makes clinical follow-up arrangements. Conducts skilled nursing assessment and planning; periodically evaluates participants and works with hospital staff and interdisciplinary teams (IDTs) to facilitate discharge plan of care to skilled nursing facilities or to participant's home or other appropriate resource.

Required qualifications:

  • Education to meet certification, license or registration requirement.
  • Upon hire: Oregon Registered Nurse License
  • 2 years Medical/surgical experience.

Preferred qualifications:

  • Recent ElderPlace, hospice, home health, utilization or care management experience.
  • Recent acute care or long term care experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
