Community Navigator Outpatient Care Management

The Community Navigator is a vital member of the outpatient care management team which strives to achieve an integrated approach to care management and community outreach and to promote, maintain and improve the health of patients and their family. Responsible for assisting patients with completing applications and registration forms, connect with transportation resources, provides appointment reminders, and works to help ensure that patients have comprehensive and coordinated care and follow-up that is continuous from initial identification through closure. They function to support the outpatient care management team with secretarial and general office work, patient navigation through the healthcare system, and other tasks to help meet goals of our population health initiatives.
Education – Graduation from high school or the equivalent required. Associate Degree preferred.
Experience – Previous clerical experience in healthcare environment preferred. Experience with office equipment to include Microsoft Office, facsimile, and electronic medical record experience preferred.
Licensure/Certification – N/A
Knowledge/Skills/Abilities – Effective communication and computer skills required. Knowledge of community resources preferred. Basic understanding of medical terminology required. Ability to prioritize work and multi-task to achieve desired results required.
