Job Summary
Summary
Evening shift pharmacist position at St Luke's Baptist Hospital.The Clinical Pharmacist is responsible for maintaining pharmaceutical dispensing functions, including clinical review functions, in accordance with all legal and regulatory requirements. The Clinical Pharmacist provides pharmaceutical care and products to all neonate, child, adolescent, adult, and geriatric patients, including inpatients and outpatients.
Responsibilities
General Functions:
- Interprets medication orders (verbal and written) and transcribes to computerized patient mediation
profiles. Maintains patient medication profiles. - Compounds and dispenses pharmaceuticals, including sterile, chemotherapy and parenteral nutrition
products - Dispenses controlled substances to patient care areas and maintains records
- Maintains the formulary by minimizing non-formulary procurements, utilizing therapeutic
substitution protocols and promoting rational drug therapy selection. - Provides direction to pharmacy personnel and ensures effective operation of the pharmacy
department. Verifies the daily activities of pharmacy technicians, interns and residents. - Complies with standards or rules and regulations of regulatory agencies: TJC, TX State Board of
Pharmacy, TX Department of Health, OSHA, DEA, TX Department of Public Safety, EEOC / TEC. - Ensures safe, appropriate, cost-effective medication therapies for patients according to established
policies, procedures and protocols 100% - Detects and reports suspected adverse drug reactions and medication occurrences.
- Utilizes formulary initiatives and clinical review to provide adequate pain management for the
patient. - Utilizes principles of drug dosing in the pediatric and neonatal patient to provide medications to
