About the role

Overview

Central Health is committed to improving community health by serving those in need. The CDCES will plan, implement, and evaluate diabetes-related education and management programs for a diverse patient population at or below 200% of the Federal Poverty Level.

 

This position focuses on providing comprehensive Diabetes Self-Management Training (DSMT) and medical nutrition therapy for adults with Type 1, Type 2, and gestational diabetes. Additionally, the CDCES® will support diabetes technology training, address food insecurity, and contribute to equitable, anti-racist care. The ideal candidate is mission-driven, detail-oriented, and skilled in multidisciplinary collaboration. This full-time clinical role includes patient education, program development, and project support across multiple Central Health sites.

Responsibilities

Essential Functions:

-Educate patients on food selection to maintain and promote health as well as to prevent and manage diabetes and associated comorbidities-Train patients with diabetes on glucose monitoring and prescribed medication use-Evaluate the nutrition and lifestyle needs of the patient, collaborate with the patient to develop and implement a plan, and document all pertinent information into the patient’s electronic medical record-Implement and monitor quality control programs related to project assignments as needed-Educate patients to use diabetes technology, including insulin pumps, smart pens, app technology, and continuous glucose monitoring (CGM) devices-Participate in or manage special projects/programs, as programs initiate/expand/or complete-Provide information and education to staff members using nutrition education materials or in-service meetings per request-May participate in or manage special projects/programs, as needed-Precepts dietetic interns as needed-Performs physical exams and malnutrition assessments for the betterment of the patients accurately in the electronic charting system-Perform other duties as assigned-Proficient in speaking, reading, and writing in Spanish (preferred)

Qualifications

Required Education:

Bachelors Degree (higher degree accepted) in Nutrition or Dietetics

 

Preferred Education:

Master's Degree (higher degree accepted) in Nutrition, Dietetics, or related field

 

Required Experience:

3 years working experience as a registered dietitian in a clinical or community setting with specialized medical care assistance (neurology, gastroenterology, endocrinology, cardiology, nephrology, pulmonology, and/or wound care)

 

Preferred Experience:

Experience in a dietitian role of making patient facing educational materials: written, handouts, or visual/video

Experience in individual and group education setting, Program Development, and bilingual English/Spanish
