About the role
SUMMARY:
Provides prescribed medical treatment and patient care services. Trains all new and current staff on clinical operations. Helps manage quality standards, PCMH, ACO and MIPS.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- LPN must have experience in Internal Medicine.
- LPN has the ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.
- LPN has the ability to plan, prepare, and teach course content and curriculum assessments and methodologies of instruction for enhancement of learning.
- LPN assigns, guides and supervises clinical staff performances in care giving to the patient of Rowan Integrated Special Needs.
- LPN orients new staff and communicates clinical objectives for the department of RISN. Evaluates current performance of clinical staff in their assigned duties.
- LPN will prepare department for all State/DOH Site Visits.
- Escorts patient to the room, preps charts, and gathers information for immediate needs of the patient. Documents patient problems, performs vital signs, records chief complaint, reviews medications, allergies, and health risk information. Prepares patients for examination.
- Answers patient calls and returns all phone messages within 24 hours (patient, family, pharmacy). Screens patients for urgency. Assists other team members and physicians to complete procedures.
- Schedules/reschedules patients’ appointments in IDX; schedules procedures; obtains pre-certifications and prior authorizations, as needed. Provides front desk coverage, including patient encounter, check-in/check-out of patients, insurance verification, and obtaining prior authorization.
- Notifies provider of abnormal test results and follows up with patient appropriately.
- Maintains patient flow. Creates, assign, and review maintain and distribute daily changes in staff schedule. Helps maintain timely workflows for physicians.
- Performs procedures efficiently and correctly within scope of practice. Acts as a Liaison between staff and providers.
- Demonstrates effective communication with patients and coworkers. Demonstrates teamwork by assisting coworkers and all members of the team with duties requested, and demonstrates initiative to learn additional skills.
- Document patient care, transition of care and continuity of care in electronic medical records.
- Contact lab /imaging facilities to obtain reports.
- Understands and adheres to Rowan’s compliance standards as they appear in Rowan’s Corporate Compliance, Code of Conduct, and Conflict of Interest policies.
- Performs other duties as assigned.
