MSW - Per Diem

Shift: 8:00am - 4:30pm

Job Description: 

PATIENT POPULATION:
The patient population served is all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY:
Reporting to the Director of Care Continuum and RN Clinical Manager and under the clinical oversight of the Licensed Clinical Social Worker (LCSW), the Masters of Social Work's (MSW) primary role is to identify and address the psychosocial needs of patients, including both inpatients and outpatients. The position acts as a resource to patients, families, and the healthcare team in removing obstacles to wellness and recover, and improving access to and coordination of services designed to improve health and safety.

Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

To perform this job successfully, an individual must have a Master’s degree in Social Work. Prior experience in the medical or acute care facility is preferred.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required. Skills in written and oral formats of communication.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

To perform this job successfully, an individual should have personal computer skills such as Windows.

LICENSURE/CERTIFICATIONS:
To perform this job successfully, an individual must have a Master’s degree in Social Work.

Responsibilities and Essential Functions:
*Indicates Essential Function

1 * Identifies the need for psychosocial assessment and intervention with high-need-risk individuals, for example those with catastrophic illness, family conflict, economic deprivation, frequent readmissions, etc. Provides and documents the indicated assessment and provides or facilitates intervention.
2 * Provides and documents regular follow up with patients assessed by MSW, sharing information with physician, nursing, and others regarding the impact of psychosocial factors on patients response to treatment, plan of care, discharge planning, etc.
3 * Accesses each patient identified as having problems with addiction and/or psychiatric disorder include suicide attempt, offers counseling and assistance, interviews family members as appropriate, and coordinates planning and referrals for aftercare. Documents each intervention.
4 Responds to staff and community concerns for child or dependent adult neglect and abuse. Evaluates each referred patient, and takes the indicated action.
5 * Remains aware of patients’ financial/insurance status and the impact of those factors on psychosocial wellbeing and availability of community resources. Provides mediating function as necessary to help patients resolve problems.
6 * Takes responsibility for convening family/staff meetings to discuss end-of-life care of other similar issues.
7 * Collaborates with Social Services Assistant, Case Manager, or others to manage complex cases, including discharge coordination.
8 * Provides follow-up calls to select patients after discharge, to assist in post hospital transition, encourage treatment adherence
