ABOUT THE JOB

Olivia Hospital & Clinic is looking for a Registered Nurse, Ambulatory Clinic. This is a full-time position working 72 hours per pay period.  This position utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and Health Partner and employer specific protocols and policy.  This position supports multiple areas within a clinic setting with strong interpersonal skills and flexibility while maintaining patient confidentiality and protecting patient rights. Resolves concerns or complaints regarding patient care or service.

 

Responsibilities:

  • Effectively assesses care needs of patients and care givers, prioritizes patient needs utilizing the nursing process; determines nursing diagnosis focused on care and identifies patient outcomes; develops patient care plan; implements and continuously evaluates the care plan; ensures patients are discharged with resolved educational needs
  • Collects and prioritizes data in a systematic and ongoing process that involves the patient, care giver, other health care providers and environment as appropriate
  • Uses appropriate evidence-based assessment techniques and instruments in data collection
  • Analyzes and interprets assessment data to determine nursing diagnoses
  • Formulates and revises nursing diagnoses that reflect the current patient status and communicates this with the patient, care giver and other colleagues within the health care team
  • Develops plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles
  • Establishes a plan that provides for continuity of care
  • Develops an individualized education plan for patients and care givers based on identified learning needs
  • Uses evidence-based research data blended with caring and compassion, to develop the plan of care and interventions
  • Coordinates, delivers, and delegates nursing care that is safe, ethical, and culturally competent based on patient assessment/standards of care in a timely manner
  • Utilizes systematic and ongoing assessment data to revise diagnoses, outcomes and the plan of care
  • Involves the patient, caregiver, and health care team members in the evaluation process when appropriate
  • Utilizes evidence-based interventions and treatments specific to the diagnoses as appropriate
  • Initiates and implements plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles
  • Provides comprehensive, individualized healthcare teaching, using the teach back method, to patients and caregivers 
  • Actively participates in creating and implementing improvements at the point of care to achieve clinical quality, patient experience, nursing engagement, efficiency, and effective outcomes
  • Evaluates the patient’s progress towards attainment of the outcomes and collaborates with members of the care team when outcomes are not as expected
  • Evaluates and documents the patient’s/care givers’ understanding of and response to the plan of care
  • Documents patient care and education accurately, timely, and completely in the EHR (electronic health record)
  • Reassesses, revises, and documents the plan of care based on the patient’s response to treatment and nursing interventions on an ongoing basis
  • Coordinates and communicates effectively with patient/care giver, clinicians, and the interdisciplinary team in a timely fashion
  • Delegates tasks consistent with the state nurse practice act based on the knowledge, skills and abilities of the designated caregiver
  • Directs, supervises, and evaluates the work of unlicensed assistive personnel in providing delegated nursing services
  • Collaborates/coordinates with other healthcare team members, patient and care givers to determine necessary resources (i.e. social services, home care, type of equipment, information) to support continuity of care

 

ABOUT YOU

  • Graduated from an accredited school of nursing.
  • Associate’s Degree or diploma required.
  • 1 year of prior experience in nursing preferred.
  • Maintain a current Minnesota registered nurse’s licensure. WI licensure where applicable.
  • BLS provider status in accordance with American Heart Association (AHA) guidelines.
  • Demonstrates knowledge of age-related growth and development necessary to provide appropriate care to the patient population served and ensure safety of patients.
  • Possesses the qualities of strong critical thinking skills and ability to multi-task.
