General Notes

This position has an end date of 03/31/2025 renewable based upon availability of funding, work performance, and progress toward programmatic goals. The Health Educator will be a member of a multidisciplinary team involving providers, community members, and research staff. This position will engage persons from vulnerable populations who may or may not be established in the healthcare system, facilitate the delivery of health education and information, coordinate care with health and social services providers in partnership with the Dell Medical School, medical homes, and community partners, connect patients with unmet needs to both social and medical resources as necessary, and data collection for health assessments. 

Provide health navigation assistance, coordinate service referrals and provide basic chronic health condition education. Mediation among individuals, communities, and health and social service systems. Provide appropriate health education and information, care coordination, and system navigation. Provide social support. Patient advocacy. Build individual and community capacity. Implement individual and or community assessments. Conduct outreach, active engagement, and recruitment.

Purpose

Work with multidisciplinary team and community partners to help patients achieve improved health practices, increase health literacy, in order to improve overall health outcomes.  

Responsibilities

  • Service Coordination, Navigation and Direct Service Skills: Connect patients to resources as needed, but not limited to medical homes and social services. Assist with follow-up appointments and care coordination. Organize and collaborate with strategic partners. 

  • Education and Facilitation Skills: Use learner-centered teaching to provide one-on-one and or small group health education and encourage patients to adopt positive lifestyle changes. Schedule and or deliver health education classes in designated communities.   

  • Individual and Community Assessment Skills; Outreach, Evaluation and Research Skills: Provide assessment and documentation of patients’ health-related social needs and assets; participate in family and community assessments. Coordinate with team members across various project requirements. Assist with developing and implement assessment tools for project longevity and maximum positive patient outcomes.  

  • Communication and Organizing Skills: In coordination with community partners, serve as liaison between Dell Medical School and other community partners. Utilize written and verbal communication, and assist with building a foundation for long term solutions. 

  • Other related duties as assigned.

Required Qualifications

High school or GED and 5 years of experience in community outreach, engagement and delivery of health education or a bachelor's degree and 1 year of experience in community outreach, engagement and delivery of health education. Successful completion of the State of Texas Community Health Worker program. Experience working with members of the community. Demonstration of interviewing techniques to obtain personal information necessary for the enrollment of services and or individuals into, and referrals, and to resolve conflicting information. Working knowledge of community-based resources and navigation including health and social services, eligibility criteria, and how to access them. Bi-lingual in English and Spanish and ability to effectively and fluently communicate verbally and in writing in both languages. Valid Texas Driver’s license. Ability to drive with a valid Texas Driver’s License and access to a reliable car to travel within Central Texas. Relevant education and experience may be substituted as appropriate.

Preferred Qualifications

3+ years of demonstrated high-level interpersonal and relationship building skills. Excellent verbal and written communication skills. Excellent community engagement skills. Strong patient-centered mindset in working with internal and external partners. Demonstrated excellence working with members of the community. Experience working for or with local health care and/or community-based organizations. Ability to use computer equipment and tablets and to learn how to use new computer software/programs.  Knowledge of HIPAA/OSHA/ADA/FERPA compliance.
